The Defense Enrollment Eligibility Reporting System (DEERS) database contains information for each Uniformed Service member (Active Duty, retired, or a member of a Reserve Component), US-sponsored foreign military, DoD and Uniformed Services civilians, other personnel as directed by the DoD (including the patient population serviced through the Military Health Services System), and their eligible family members. DEERS registration is required for TRICARE eligibility and enrollment. Incorrect information in the DEERS database can cause problems with TRICARE claims as well as other health care benefits, so it is critical to maintain your DEERS information.
Proper identification is required for all transactions. To be issued any ID card, 2 forms of unexpired identification are required for adults, one of which must be a current photo ID. CHILDREN under 18 years of age, only one form of ID is needed. Notarized English translation must accompany foreign documents.
